Rejuvenate Clinic - General Terms & Conditions
1. About These Terms
These terms apply to bookings, services and products supplied by Rejuvenate By Tash Pty Ltd, trading as Rejuvenate Clinic, ACN 655 686 507. Our clinic is at 274 Ross River Road, Aitkenvale QLD 4814.
Contact us on 0406 965 605 or info@rejuvenateclinic.au.
Â
Effective date: 16 October 2026. We provide these terms and any relevant booking or purchase conditions before you commit. They do not replace individual clinical advice, informed consent or rights that cannot be excluded by law. Acceptance of booking terms is not consent to a medical procedure.
Â
2. Assessment and Informed Choice
All procedures are subject to an individual assessment by an appropriately qualified practitioner. Booking an appointment does not guarantee that treatment will be recommended, prescribed or performed. Additional assessment or referral may be required.
Before you decide, your practitioner will explain the proposed care, expected benefits, material risks, limitations, alternatives including no treatment, recovery, follow-up and costs. You may ask questions, seek another opinion, take time to decide or withdraw consent before treatment. Any consultation or other service already provided remains payable at the fee disclosed in advance.
We do not provide elective cosmetic procedures to people under 18. Clinical decisions and required waiting periods take priority over appointment availability or promotional conditions.
3. Health Information and Suitability
Please provide accurate information to the best of your knowledge and tell us about relevant medicines, allergies, health conditions, previous procedures and changes since your last visit. Ask us if you are unsure what is relevant. We may defer treatment if information needed for safe care is unavailable. This does not remove our professional obligations or your legal rights.
4. Results and Aftercare
Results, recovery and the number or frequency of sessions vary. We do not promise a particular appearance, complete removal of a concern or a fixed duration of benefit. Further treatment is recommended only after assessment.
We explain aftercare and any clinically necessary products or supplies, including their costs, before you consent. Optional skincare purchases are voluntary. If a product is necessary for safe care, we will explain why, discuss suitable alternatives and defer treatment if appropriate. Declining a particular brand does not waive your rights.
5. Fees and Quotations
Amounts are in Australian dollars and include GST where applicable. We disclose consultation fees, deposits, treatment charges and any separate practitioner or review fees before they are incurred. Quotations explain their scope and any foreseeable additional costs. Changes require your agreement before further non-urgent services are supplied. New prices do not retrospectively change a confirmed prepaid purchase.
6. Consultation Payments and Deposits
A consultation fee pays for the assessment and advice provided, whether or not treatment follows. Its amount and any credit towards later care are disclosed before booking. A deposit is an advance payment towards the agreed service and is credited to that service. We identify which type of payment is required before collecting it.
Where you cancel at least 48 hours before the appointment, we will refund the advance payment for services not provided, or transfer it if you prefer. Any lawful late-cancellation charge is dealt with under section 7. We do not require you to purchase a product or have treatment to avoid an undisclosed fee.
Â
7. Changes and Missed Appointments
Please give at least 48 hours notice to cancel or reschedule. Contact us by telephone or email, or use the online portal where available. We assess notice by when it is received, rather than when staff read it. Keep your cancellation confirmation.
For less than 48 hours notice or a missed appointment, a fee may apply up to the cap disclosed for that appointment: $100. It will not exceed our reasonable net loss from the booking, after allowing for costs saved and replacement bookings. Any retained deposit is counted towards this amount; it is not an extra charge. We provide an explanation on request.
We consider illness, emergencies and other exceptional circumstances individually. Please contact us promptly. Failure to respond to a reminder alone does not cause deposit forfeiture. If late arrival prevents safe care, we will discuss available options and any proportionate charge under this policy.
If you confirm your appointment and you do not attend your appointment a $100 "no-show fee" will be deducted from your nominated card that was used for your booking deposit via our third party provider stripe. Booking Portal link as follows to change your appointment: https://clientportal.powerdiary.com/clientportal/rejuvenateclinic
8. Changes Made by the Clinic
If we cancel or cannot provide a booked service, you may choose a suitable alternative appointment or a refund of the advance payment for that service. We will not require you to accept credit instead of a refund where a refund is due. Other rights available under law are unaffected.
9. Card Payments
The booking or payment screen identifies the payment provider, payment purpose and amount. Any card-on-file or recurring payment authority must be clearly disclosed and separately authorised where required. We do not treat acceptance of these general terms as unlimited authority to charge your card.
Any cancellation charge must follow the disclosed policy and your payment authority. We notify you of the amount and basis before collecting it. Contact us to query a payment or discuss withdrawal of future payment authority; withdrawing authority does not by itself cancel an existing lawful debt.
10. Product Use
Follow the manufacturer instructions and your treating practitioner advice. Tell us about known allergies and seek advice if unsure whether a product is suitable. Patch testing, when recommended, does not guarantee that a reaction will not occur. Stop use and seek advice if you experience a suspected reaction. No product-use instruction excludes your consumer rights or our responsibility under law.
11. Consumer Rights and Refunds
Our goods and services come with guarantees under Australian Consumer Law that cannot be excluded. These include applicable guarantees about acceptable product quality and services provided with due care and skill. You may be entitled to a repair, replacement, refund, cancellation, compensation or another remedy, depending on the circumstances. We will assess concerns fairly and provide the remedy required by law.
A change of mind alone does not generally create a statutory refund right. For completed services and products supplied as agreed without a consumer-guarantee failure, we do not ordinarily offer change-of-mind refunds. Advance payments, courses and memberships are addressed separately below. Sale prices, opened packaging and known procedure risks do not automatically remove rights concerning a faulty product or deficient service.
An adverse outcome does not automatically establish a failure of care; equally, a signed consent form or disclosed risk does not automatically defeat a valid claim. We will not require you to waive legal rights to have a concern assessed.
12. Adverse Events and Follow Up
Contact us promptly about unexpected symptoms or concerns after treatment. Do not wait for a scheduled review. We arrange assessment or referral according to clinical urgency. If you cannot reach us and need urgent care, contact an appropriate urgent medical service. For a medical emergency, call 000.
You will receive procedure-specific aftercare and after-hours instructions. Routine review timing does not limit when you can seek help. We will explain any review or further-care costs in advance where practicable. Charges do not override any entitlement to a remedy under law.
13. Complaints
Please contact the clinic manager on 0406 965 605 or info@rejuvenateclinic.au. We will acknowledge your complaint promptly, assess immediate safety concerns and explain the next steps and expected response time. We will update you if further investigation is needed. You are not required to wait a fixed investigation period before seeking independent advice or raising a complaint externally.
You may contact the Queensland Office of the Health Ombudsman about a health service complaint at www.oho.qld.gov.au/make-a-complaint or 133 646. You may also seek assistance from the Queensland Office of Fair Trading for consumer matters or the Office of the Australian Information Commissioner for privacy concerns.
Raising a complaint will not result in retaliation or loss of statutory rights.
14. Liability
Nothing in these terms excludes, restricts or modifies consumer guarantees, liability for negligence or other rights and obligations that cannot lawfully be excluded. Agreeing to these terms or to clinical consent does not release us from those obligations. Responsibility for loss will be determined under applicable law; no blanket patient indemnity or general monetary liability cap is imposed by these terms.
15. Prepaid Courses
A prepaid course is available only where lawful and clinically appropriate. Before purchase we specify the included services, total price, allocated value of each session, validity period, exclusions and cancellation conditions. A course is not a commitment to undergo a procedure that later becomes unsuitable. Treatment timing follows individual assessment.
If treatment becomes clinically unsuitable or we cannot provide the remaining services, we refund the unused amount, allowing for any services already properly supplied. We will not force you to accept another treatment.
For other requests to end a course, we refund unused value less any reasonable, disclosed and lawful cancellation costs. Past sessions are not retrospectively repriced to a higher rate.
Any expiry is stated before purchase and remains subject to applicable gift-card laws and consumer rights. We consider extensions for clinical deferral, clinic unavailability or other reasonable circumstances. Expiry does not extinguish legal remedies. Existing courses are assessed under their original lawful terms.
16. Gift Vouchers
The voucher states its value, eligible products or services, issue date and expiry. Standard purchased vouchers are valid for at least three years from supply or purchase and for any longer period stated. Restrictions are disclosed before sale. A voucher does not guarantee clinical suitability or priority access, and the recipient remains free to decline treatment.
We do not charge prohibited post-supply fees. Remaining value is available until the stated lawful expiry. Vouchers are not ordinarily redeemable for cash for change of mind, but refunds and other remedies required by law remain available. If we cannot supply the eligible goods or services, we will provide an appropriate remedy.
Keep vouchers secure and report loss promptly. Where ownership and unused value can be verified, we will consider replacement using reasonable security checks. We do not cancel valid paid value merely because a voucher programme ends.
17. Privacy and Photographs
We collect and handle personal and health information in accordance with applicable privacy law and our Privacy Policy. We explain relevant collection purposes and disclosures. Clinical communications and reminders are separate from promotional communications. We obtain consent where required for direct marketing, and you may opt out without affecting your care.
Clinical photography and any proposed publication are explained separately. Agreement to treatment or clinical photographs does not authorise advertising use. Publication requires separate, specific consent and must itself be lawful. You may withdraw permission for future marketing use; we will explain any practical limits concerning material already distributed.
18. Attendance and Ending Services
Please arrange supervision for accompanying children; clinic staff do not provide childcare. Some treatment spaces cannot safely accommodate children. Discuss arrangements before attending. This does not exclude our legal responsibilities for premises safety.
We may decline or discontinue a service on reasonable clinical, safety, lawful scope-of-practice or serious conduct grounds. Where practicable, we explain the reason, discuss alternatives and arrange appropriate continuity of care. Unused payments and other rights are handled under these terms and applicable law.
19. Website Information and Intellectual Property
Website information is general and does not replace individual assessment. We aim to keep it accurate and current and correct material errors when identified. This statement does not excuse misleading advertising or remove our responsibility for professional advice.
Website content belongs to us or its respective rights holders. You may view and download it for personal information and any use permitted by law. Other reproduction requires appropriate permission. Third-party links are provided for convenience; their inclusion does not change your rights against us.
20. Updates and Governing Law
We may update these terms for future bookings and purchases by publishing a new effective date. We do not retrospectively change an agreed purchase or membership without a lawful basis and any required consent. We retain earlier versions on request. If a term is invalid, the remaining terms continue so far as legally possible.
Queensland and applicable Commonwealth laws govern these terms. Nothing restricts your right to use a court, tribunal, regulator or complaint process available under law.
Â
21. Provision Of InformationÂ
In order for us to provide our Products and Treatments to you, you may be required to complete and submit medical history and client consent information/forms. In some cases, we may be able to provide a limited range of Treatments to you if you do not wish to provide the information required to complete a client consent form, such as your medical history, however we retain the right to refuse to provide our Products and Treatments to you in our absolute discretion.
In order to complete the client consent form and medical history forms, you will be required to provide us with a range of information about yourself including without limitation information about your health, medical conditions, treatment history, family health history, diet and lifestyle. Â
22. Privacy

We collect personal information when a Product or Treatment is purchased in order to provide the requested treatment, provide updated information, and advise which other Treatments may be of interest to you. You do not need to provide us with personal information, however this may result in us being unable to provide a Product or Treatment to you.

You must ensure that the personal information you provide is accurate and complete and that all ordering or registration details (where applicable) contain your correct name, address and other requested details. You agree not to provide misleading, deceptive or false information and that you indemnify us against any claims made against us as a result of your non-compliance with these terms.

We may collect information disclosed to us about your physical and mental health and well-being (‘Sensitive Information’).
We will only ever use Sensitive Information for providing Treatments to customers. Sensitive Information will not be used or disclosed for any other purpose without consent, except where disclosure may be required by law or is necessary to protect the rights or property of Rejuvenate – Cosmetic, Health & Wellness Clinic, a related company, any member of the public, or to lessen a serious threat to a person’s health or safety.
